Chronic Illnesses and Chronic Care Management Program

We have a dedicated nurse assigned to our Chronic Care Management Program to help you better manage these conditions and keep them from getting worse. Her goal is to be sure you are taking your medications and taking them as prescribed, to have less hospitalizations, to be sure you are keeping your appointments with specialists', and to answer questions about your chronic conditions.  Your physician and primary care team will carefully monitor and provide comprehensive care for your chronic health conditions in a systematic way to supplement regular office visit care.
